Guidance for submitting behavioral health appeals

Evernorth Behavioral Health (Evernorth®) is committed to making it easier for providers to work with us. A recent review of appeal requests identified an opportunity to offer clearer guidance on the appeals process and support more timely, efficient handling.

To help prevent delays, appeals for behavioral health services should be submitted directly to Evernorth, rather than Cigna Healthcare®.

When to submit an appeal

The following circumstances may prompt an appeal request:

  • Administrative denials for mental health services, substance use disorder services, and any other services covered under behavioral health benefits.
  • Medical necessity or inpatient facility denials related to level of care, length of stay, or delayed treatment days.
  • Claim denial related to untimely claims submission (failure to submit a claim within 90 days of the date of service).

How to request an appeal

To facilitate an appeal, submit a completed Behavioral Appeals Cover Sheet Form to Evernorth containing a detailed description of why you are appealing the decision.

  • To help ensure timely processing, make sure all fields of the form are complete, and include any supporting documentation (medical records, bills, related correspondence) with your appeal submission.
