2027 Cigna Care Designation and Tier 1 results

Cigna Healthcare® evaluates in-network providers in 21 specialty types, including three primary care and 18 specialties. Provider groups that are identified as higher performing for quality and cost efficiency relative to their peers may earn Cigna® Care Designation (CCD).

CCD information may also be used to help determine a provider’s tier for plans with a tiered benefit design. Customers enrolled in a plan with tiered benefits may pay lower copayments or coinsurance for covered services rendered by Tier 1 providers.

In prior years, CCD was assigned at the group Taxpayer Identification Number (TIN) level, even though quality and cost efficiency were measured by specialty. Beginning with the 2027 CCD program, providers will continue to be aligned to a practice based on TIN, but primary care providers (PCPs) and specialty care providers (SCPs) will be evaluated as separate groups using methodologies tailored to the type of care they deliver.

Communications for 2027 CCD

In June, PCPs and SCPs will receive communications explaining how to access their 2027 quality, cost efficiency, and CCD directory display results.*

2027 CCD and Tier 1 provider directory displays

  • October 2026: Providers who receive the 2027 Tier 1 status will have a “Tier 1 Provider” indicator display next to their name in our provider directories on Cigna.com and myCigna.com®.
  • January 1, 2027: The 2027 CCD and Tier 1 provider status will become effective.
  • Providers who earned the 2027 CCD will have the CCD symbol  display next to their name in our online directories.
  • 2027 quality displays will be available on Cigna.com and myCigna.com.
  • 2027 cost efficiency information will only be available on myCigna.com.

Important dates and timing

  • August 17, 2026: Deadline to submit CCD reconsideration requests if you want any approved changes to appear in the October 2026 online directory preview.
  • October 2026: Online provider directories will display Tier 1 status only, as a preview for the upcoming program year.
  • January 1, 2027: 2027 Quality, cost‑efficiency, CCD results, and final Tier 1 provider status become effective and display in online provider directories.
  • CCD reconsideration requests received after August 17, 2026: Requests will still be reviewed; however, approved updates will not appear in the October preview and will instead be reflected either January 1, 2027, or in the next applicable monthly update.

Quality Clinical Managers (QCMs) are dedicated to supporting the provider or provider group throughout this process. They act as your advocate and partner, working with you to explain your results, review the underlying data, and addressing any questions or concerns. We are committed to working together to promote understanding and transparency.

Providers or provider groups may submit a request to the QCM team using the secure form at Cigna.com/CCDRequest to:

  • Obtain your 2027 Practice Summary Report
  • Request a teleconference meeting to discuss the results
  • Request additional quality and cost-efficiency information
  • Obtain a full description of the methodology and data used
  • Request correction of potential data inaccuracies
  • Request reconsideration of CCD results
  • Submit additional or supporting information
