Cigna Healthcare Expands Personalized Care Management to Help More Customers Access Support Earlier

More than 1,250 Cigna Healthcare-employed clinicians, including nurse case managers and behavioral health specialists, are helping customers navigate the health care system, coordinate services, and access additional resources while managing emerging, complex, or chronic health needs – including cancer, heart disease, kidney disease, high-risk pregnancy, and behavioral health conditions.

Building on this personalized approach to care, Cigna Healthcare is expanding its care management programs to help more customers access support earlier in their health journeys. New AI-enabled capabilities and predictive analytics will help care teams identify customers who may benefit from additional support sooner and connect them with care management resources. These capabilities are designed to support earlier outreach and engagement, while treatment decisions remain between customers and their treating providers.

The expanded program will support 20% more customers, helping them better understand and follow their providers’ care plans, coordinate services, and address barriers to treatment that may affect their health. Among customers who engage in care management, these programs are expected to reduce medical costs by an average of $2,000 per year.

As more customers are connected to care management earlier, Cigna Healthcare clinicians will continue collaborating with treating providers to reinforce care plans, coordinate care, and connect customers with clinical, behavioral, and community resources. By supporting customers between provider visits and helping address barriers to care that may affect health outcomes, these efforts can contribute to a more connected care experience and better health and financial outcomes for your patients.
