Decoding patient ID cards: A quick guide for providers

June 8, 2026

We pack a lot of important information on our ID cards. When your patients with Cigna Healthcare® coverage present you with their ID card, either a physical or digital version, having the plan information you need at your fingertips allows you to quickly and efficiently serve them.

Our ID card brochure defines and clarifies the information that appears on our most common ID cards and outlines the requirements associated with our various plans, including managed care, Individual & Family Plans, Cigna Global Health Benefits®, Cigna Choice Fund®, Shared Administration Repricing, strategic alliance, Payer Solutions, and indemnity.

What you can expect

Throughout the brochure, you will find descriptions of our available plans, including referral requirements, coverage details, and primary care provider requirements, as well as sample standard ID card images for each plan. Each sample ID card has a corresponding key to help you more easily identify the various fields.

In addition, since many of your patients may be presenting you with a digital version of their ID card, the brochure has a dedicated section about digital ID cards, which includes a sample of what your patients may show you.

As a reminder, all images in the guide are samples, and the actual content on a patient’s ID card may vary to conform to a state’s legislative and regulatory requirements. An ID card is not a guarantee of coverage, and benefits should be verified.

How to access the guide

The ID card brochure is available on the Cigna for Health Care Professionals website (CignaforHCP.com). We may occasionally update this brochure during the year. To ensure you always have the most up-to-date information, we encourage you to bookmark the Using ID Cards page on CignaforHCP.com.
