It’s the silent killer1 and the leading cause of death for men, women, and people of most racial and ethnic groups.2

The statistics highlight the increasingly complex challenge of identifying and caring for patients at risk for heart disease. From managing multiple chronic conditions to addressing social determinants of health, the need for preventing and managing heart health continues to grow.

To help meet these challenges, Cigna Healthcare® has partnered with Heartbeat Health, a virtual cardiology care provider, to offer enhanced support, patient education, and care coordination.

A virtual cardiology partner

Heartbeat Health delivers virtual cardiac care services designed to reduce barriers and accelerate access to care. Their model combines virtual visits with comprehensive evaluations, monitoring, and diagnostics to support your treatment plan and improve patient adherence.

Services offered

  • Same or next-day virtual appointments with a cardiologist or advanced practitioner for patient assessment.
  • Order diagnostics, such as laboratory testing, or conduct home-based testing (e.g., remote electrocardiogram).
  • Prescribe or titrate medications (such as statins) and refer back to PCP for continued medication management.
  • Documentation of any adverse reactions in the patient health record.
  • Patient education to improve adherence and reduce gaps.
  • Monitoring and follow-up calls with patients to ensure that prescriptions have been filled and taken as directed.

Added value for providers

Heartbeat Health acts as an extension of your practice, supporting your relationship with your patient to address immediate cardiovascular needs, then transitions the patient back to you as soon as medically appropriate. They can:

  • Assist in early detection and identification of undiagnosed heart disease.
  • Enhance access for patients with limited mobility, transportation challenges, or geographic barriers.
  • Reduce or avoid unnecessary trips to the emergency room or hospital readmissions.
  • Drive patient adherence to a treatment plan through frequent touchpoints and education between in-person visits.
  • Coordinate to share information from virtual visits. PCPs will receive clinical documentation and updates with 24 hours of the patient encounter through a secure eFax channel. Visit notes include a summary of the visit, as well as laboratory testing and diagnostics ordered.

Patient participation and enrollment

Patients with Cigna Healthcare coverage, including those aligned to Cigna Collaborative Accountable Care (CAC) programs*, are eligible for support through Heartbeat Health.** Providers may refer patients directly, or patients can self-enroll. To get started:

*Cigna CAC programs work with provider Groups to deliver coordinated, quality, and cost-efficient care for better health outcomes and savings.

** Standard copayments and deductibles for virtual care visits apply, but can vary according to benefit plan coverage. Patients should consult their individual benefit plan for specific coverage details.
