Whether it’s for one patient or up to 100, you can use the Coverage Confirmation Grid (an Excel spreadsheet) to verify general benefits and eligibility, including if a specific provider is in-network, prior authorization requirements, and accumulation information. If you provide employee assistance program (EAP) services, you can also use the grid to request new and/or verify existing EAP prior authorizations.

Upon securely emailing your Coverage Confirmation Grid to Evernorth, we will review it and respond within 24 business hours from the date the grid was received.
