Case management treatment plan requests help support patient care

To help support your care of patients with Cigna Healthcare® plans who have certain medical conditions, we offer holistic support through a comprehensive case management program. The program focuses on advocating for the health care provider’s current treatment plan, assisting with navigation of health care benefits, connecting patients to additional resources, and addressing barriers to care.

As part of their participation in the case management program, your patients are required to have a periodic care plan on file so that case managers can provide the most effective support.

That’s why, when your patient enrolls in the program, your practice will receive a fax requesting that an attached treatment plan form be completed and returned to the case manager. If applicable, the fax request will also list any identified gaps in care for your awareness and collaboration.

Sending the form back in a timely manner helps us to collaborate better with you to support your care and treatment plans, increase your patient’s engagement, and improve their health outcomes. If it’s more convenient, you may send a clinical summary instead with any additional information you feel is relevant to the patient’s plan of care.

It is important to know that our treatment plan request is in compliance with the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule, which permits the use and disclosure of protected health information without patient authorization for treatment, payment, and health care operations, including the management of care.
