Interwell Health^® is a value-based kidney care company that helps support dialysis facilities manage the care of their patients undergoing dialysis treatment. Since 2017, Cigna Healthcare^℠ has partnered with them to provide a condition management program for customers with end-stage renal disease (ESRD) who receive dialysis treatments at a Fresenius Kidney Care facility. (Cigna Healthcare also provides a value-based condition management program for customers with ESRD who receive dialysis treatments at a DaVita^® Kidney Care facility.)

This month, we will expand our partnership with Interwell Health to provide an ESRD condition management program for our customers who are not covered under a value-based care program.

About the program

Interwell engages directly with patients to coordinate scheduling of and transportation to appointments, provides coaching to help them adhere to their care plan, offers access to supportive resources, and helps them better manage their health.

In addition, Interwell implements a risk-stratification process to identify patients who are at a high risk of hospitalization. These patients will receive targeted interventions to manage their condition more effectively and reduce the likelihood of emergency room visits and inpatient stays.

The program is available to eligible customers at no cost, and does not affect their ability to participate in other Cigna Healthcare coaching or case management programs.
