Beginning in August, the Healthmap Solutions Chronic Kidney Disease (CKD) Program will be available for eligible patients with Cigna Healthcare^℠ coverage in 27 new markets. The program is administered by Healthmap Solutions, an industry-leading National Committee for Quality Assurance-accredited kidney population health management company, and expands our current kidney care solutions to focus on disease management for vulnerable patients experiencing pre-dialysis.

The CKD program is currently offered in Florida, Georgia, North Carolina, and South Carolina. The program will now be available to eligible customers in these additional markets:

About the program

Healthmap Solutions will contact eligible patients with stage 4 or 5 CKD and invite them to participate in the program, which is available to them at no cost. The CKD program will help to:

• Improve clinical outcomes, including the use of evidence-based approaches to delay disease progression
• Reduce emergency room visits and inpatient admissions
• Identify gaps in care and align a patient’s plan of care
• Share patient-specific recommendations on a monthly basis

Participation in the CKD program will not affect a patient’s ability to participate in other Cigna Healthcare coaching or case management programs.
