Healthmap Solutions Chronic Kidney Disease Program will reach customers in 27 additional markets

Beginning in August, the Healthmap Solutions Chronic Kidney Disease (CKD) Program will be available for eligible patients with Cigna Healthcare coverage in 27 new markets. The program is administered by Healthmap Solutions, an industry-leading National Committee for Quality Assurance-accredited kidney population health management company, and expands our current kidney care solutions to focus on disease management for vulnerable patients experiencing pre-dialysis.

Market expansion

The CKD program is currently offered in Florida, Georgia, North Carolina, and South Carolina. The program will now be available to eligible customers in these additional markets:

About the program

Healthmap Solutions will contact eligible patients with stage 4 or 5 CKD and invite them to participate in the program, which is available to them at no cost. The CKD program will help to:

  • Improve clinical outcomes, including the use of evidence-based approaches to delay disease progression
  • Reduce emergency room visits and inpatient admissions
  • Identify gaps in care and align a patient’s plan of care
  • Share patient-specific recommendations on a monthly basis

Participation in the CKD program will not affect a patient’s ability to participate in other Cigna Healthcare coaching or case management programs.
