New rejection codes for Medicare Advantage claims

On December 5, our claim processing system for Cigna HealthcareSM Medicare Advantage plans will be updated to include new rejection codes. This may result in claim rejections due to mismatched customer information.

You should review your patient’s information and resubmit a corrected claim if you see one of the following rejection codes:

  • A3: Acknowledgement/Returned as un-processable claim (the claim/encounter has been rejected and has not been entered into the adjudication system)
  • 31: Subscriber and policyholder name mismatched
  • IL: Insured or Subscriber
