When allergy information is easy to find and clearly documented in the medical record, providers can move through patient care with fewer interruptions. The record should note known allergies and describe the type and severity of each reaction—such as rash, hives, or anaphylaxis—so that key details are available when they are needed most. As part of our quality improvement program, Cigna Healthcare® conducts annual medical record reviews in selected markets to assess documentation, including drug allergies and related adverse reactions. The review looks for allergy information that is clearly documented, readable, and easy to locate—helping support provider readiness and a more efficient review process.
